This policy describes how Macula Networks ("we," "us") collects, uses, and protects information submitted through this website, maculanetworks.com. It is written to be read, not skimmed past — if anything here is unclear, ask us.
The most important thing first: no patient information
This website does not collect, and is not designed to receive, protected health information (PHI). The forms on this site collect business contact details and practice-level answers only. Please do not enter patient names, records, or any other patient information into any form on this site.
If your practice becomes a client, any patient information involved in delivering services is governed by our service agreements and business associate agreement with you — not by this website policy.
What we collect
We collect only what you choose to submit, plus standard website analytics:
- Contact form: your name, email address, phone number (optional), company/practice name (optional), and your message.
- HIPAA self-assessment: your email address, practice name (optional), and your practice-level answers to the assessment questions. The assessment asks about administrative safeguards — it does not ask about, or accept, patient data.
- Book a Call: your name, email address, phone number, practice name (optional), your requested time, and an optional note.
- Client reviews: if you submit a review through an invitation we send you, we collect the review content, rating, and the name and details you choose to display.
- Analytics: we use Google Analytics to understand how the site is used — pages visited, approximate location (city level), device type, and how you arrived. This uses cookies. We do not use advertising cookies and we do not run ads.
How we use it
- To respond to your inquiry, schedule your call, or deliver your assessment results.
- To follow up about the services you asked about. If you take the self-assessment, we may email you about your results and how we can help close the gaps it found.
- To publish reviews you have chosen to share.
- To improve the website.
We do not sell your information. We do not share it with third parties for their marketing. We do not send bulk marketing email to lists — you will hear from a person, about the thing you asked about.
Where your information goes
Form submissions are transmitted securely to our own client-management system. Limited service providers process data on our behalf — website hosting, and Google Analytics as described above. These providers process data only to provide their service to us.
Data retention
We keep form submissions as long as they are useful for the purpose you submitted them — an inquiry we are responding to, an assessment we are following up on, or a client relationship. If you want something deleted, ask (below), and unless we have a legal reason to keep it, we will delete it.
Security
The site is served over HTTPS, and form submissions are encrypted in transit. We limit access to submitted information to the people who need it to respond to you. Security is our business; we hold ourselves to the standard we hold our clients to.
Your choices
- Analytics: you can block analytics cookies in your browser, or use Google's opt-out browser add-on. The site works fully without analytics.
- Access and deletion: email us at info@maculanetworks.com to ask what we hold about you, correct it, or have it deleted.
- Email: any follow-up email we send includes a working way to say "stop" — and we will.
Children
This site is for optometry practice owners and staff. It is not directed to children, and we do not knowingly collect information from anyone under 18.
Changes to this policy
If we change this policy, we will update the effective date at the top of this page. Material changes will be noted plainly, not buried.
